fix: prevent Ecto.ConstraintError on audit_log pkey during log_changes (OPS-4620)

+- `*_and_log/*` and `log/*` no longer raise `Ecto.ConstraintError` on `audit_logs_pkey` (or custom table pkey) when a duplicate PK is generated for the audit row. The `changelog_changeset` now declares `unique_constraint(:id)` so the log failure is turned into a changeset error that the existing log_* paths swallow (main tx succeeds). Added regression test. Closes OPS-4620.
