fix: prevent Ecto.ConstraintError on audit_logs_pkey during *_and_log (OPS-4618)

+- `update_and_log/4`, `insert_and_log/4` (and siblings) no longer raise `Ecto.ConstraintError` on `audit_log_pkey` when the audit insert races or collides on the primary key. The log insert now declares `unique_constraint(:id, name: "<table>_pkey")` so the violation becomes a changeset error that is swallowed (best-effort logging). The main operation succeeds. Closes OPS-4618.
