+- Ecto.ConstraintError on audit_logs_pkey (and custom table pkey) originating in log_changes/update_and_log and friends. Added `unique_constraint/3` on the pkey name (derived from :table_name config, default "audit_log") so the violation becomes a changeset error (soft-failed and logged, preserving prior behavior) instead of raising. Closes OPS-4590.
