Daily cross-repo compat checks surfaced a recurring schema error: custom MCP servers were declared directly under tools: (e.g., serena, tavily) instead of mcp-servers:. This PR adds an automatic gh aw fix codemod to normalize that pattern.

What changed
- Added new codemod: tools-unknown-to-mcp-servers
  - Migrates unknown tools.<name> entries to mcp-servers.<name>
  - Preserves recognized built-in tools entries unchanged
- Registered codemod in the fix pipeline so gh aw fix --write applies it automatically.
Migration behavior
- If value is a list, wraps it under server tools:
- If server config has no command, injects:
  - command: "..." # TODO: fill in command
- If mcp-servers: already exists, appends migrated entries there.
Tests added/updated
- Added focused codemod tests for:
  - Serena-style list input
  - Tavily-style object input
  - No-op on built-in tools
  - Append into existing mcp-servers
- Updated codemod registry tests for count/order/id coverage.
