Promotes CHANGELOG's [Unreleased] section (OpenCode plugin, Codex CLI plugin, consolidate-memory, three bug fixes, this session's Codex hooks investigation, and the docs/promo-site redesign) to [0.13.0], and bumps both .claude-plugin/plugin.json and .codex-plugin/plugin.json to match. Also backfilled the missing v0.7.0-v0.11.0 tags and GitHub releases (tags existed as commits but were never tagged/released) and created a v0.12.0 release (tag existed, no release object) - the releases page previously stopped at v0.6.1 despite six shipped versions since.
