Uses strict `O_NOFOLLOW | O_CLOEXEC | O_CREAT | O_WRONLY | O_APPEND` flags to explicitly reject symlinks and enforce secure 0600 file permissions, preventing TOCTOU symlink attacks during log creation/appending.
