Register Operations MCP from Codex and Claude plugins

Summary

• release the plugin layer as 0.1.1 while keeping core and MCP npm artifacts pinned at 0.1.0
• add generated Codex and Claude .mcp.json configs for the immutable stdio package
• update manifests, marketplace versions, skill guidance, and host refresh / database-path guidance
• add fail-closed drift checks for plugin versions and MCP commands
