Add optional anonymous usage telemetry to Dify plugin

Collect anonymous metrics on each plugin invocation with optional user opt-in.

Metrics to collect (JSON):

• dialect (SQL dialect used)
• result (allowed/blocked/fixed)
• violation_type (if blocked: column, table, or other)

User can enable/disable telemetry via configuration. No personal data collected.
