summarizeDemographics does not stratify alive/dead patients

summarizeDemographics.R does not split/rollup summary for both alive and dead patients. This issue is a request to include a summary of number of alive/dead patients in the pulled cohort, and to add a stratification in the various demographics summary numbers.
